Support Coordination

• We help with assisting you to managing your budget for each type of support and negotiating prices and service agreements that is within

• Communicate with careers and families to ensure they are aware of rights and responsibilities to make informed choices.

• Guide participants and supporters through the intake process, assisting to follow the process for NDIS planning

• Develop and maintain positive relations with participants during the length of relationship between Collaborative Supports and their participants process, including the ongoing provision service

• We also Coordinate complex support and services which are developed and delivered within the funding plan

• Develop a support plan that identifies clear actions

• We always ensure support and service is developed and delivered according to the wishes of each participant and their families

• Point of contact between your service providers and allied health professionals

• We will research and help to recommend service providers that suit your needs

•  Experienced in mental health, intellectual disability, positive behaviour support and restrictive practices

• Maintaining professional relationships and communication with internal and external providers and allied health care professionals as well as government agencies relevant to your needs on your behalf sand requests

• Will always assist you in planning ahead to prepare for your plan review.

• We are obliged to manage conflict of interest; we will recommend service providers that are most suited your needs and goals.

• Assisting participants to achieve goals through building capacity, knowledge, resources, and confidence.
